A 62-year old man with a 35 pack/year smoking history had his left lower lobe resected because of a small-cell carcinoma.When he arrives to recovery room he is stable with an 8.0 ET tube and stable vitals. Approximately 60 minutes after his arrival the high pressure on the ventilator is sounding, terminating the breath. His vitals are as follows: BP 140/90, HR 118, RR 12 (ventilator assisted) and his SpO2 92%. Current ventilator setting are as follows – PRVC, VT 450 mL, RR 12, PEEP 5, FiO2 40%, Flow 60 lpm, Pip 50, and Plat 18. A 24-year old male, pedestrian struck is brought to the ER. He currently has marked diminished breaths sounds on his right side compounded with rebound tenderness. X-ray reveals that he has a tension pneumothorax on his right side with broken ribs and a compound broken left femur. He is intubated with an 8 ET tube on arrival. Prior to chest tube insertion it should be noted that the ventilator is alarming high pressure. His are vitals are as follows: BP 110/90, HR 130, RR 12 (ventilator assisted),temperature of 98.1°F, and his SpO2 96%. Current ventilator settings are as follows: Mode – PRVC, VT 500 mL, RR 12, PEEP 5, FiO2 100%, Flow 70 lpm, Pip 50, and Plat 28.

She intubated with a 7.0 ET tube and her breath sounds indicate crackles midway up bilaterally. Over the last 24 hours it should be acknowledge that the patient’s urinary output has decreased. Currently, minimal secretions are being suctioned Q 4 hrs. Vitals are as follows: BP 100/60, HR 94, RR 10 (ventilator assisted), 2 spontaneous breaths, temperature of 101°F, and her SpO2 98%. Her currently ventilator settings are as follows: Mode – AC, VT 350 mL, RR 10, PEEP 6, FiO2 60%, Flow 60 lpm, Pip 32, and Plat 24.
