Physical Education Grant Application

Action for Healthy Schools works with schools to guarantee that every studnet is healthy. They offer three grants, two of which are specific to sports The Game on Grant, and the Game On for Parents grant. These grants support school PE as well as after school athletics and other school programs. Game on grants are for teachers and administrators and the Game On for Parents are predominantly. About the Grant Funds can be used towards the purchase of equipment, evaluation costs (printing/copying), or incentives ( no more than 10% of the total budget. ) Grants range from 500 to 5,000, with most schools receiving a full 5,000. You can use the grant money for: Improving Facilities used for sports Buying equipment for recess Improving or creating playgrounds/play-spaces Physical education program ( but money cannot go towards administrative costs like salaries or stipends) Intramural and/or before/after-school programs that promote a healthy and active lifestyle. Nutrition initiatives such as nutrition education and gardening  Cover letter Introduction to project. Address the letter to (Charles Zimmerman) Include how much money I’m requesting and a summary of what it is exactly that thew Action for Healthy Kids will be funding. Executive Summary: Describe your organization, State the organization’s mission, How does the project fit that mission?    Statement of need Clearly define the specific problem you are seeking to solve, include research Goals and Objectives: What are the outcomes of the project? What are the outcomes of the project? What will you achieve in a general and specific manner? Refer to research  Methods and Strategies: In detail describe how the project will solve the problem and reach the goals and objectives. (A most important part of the application.) Plan Evaluation: How will you evaluate the project on an ongoing basis to ensure that the money is being used efficiently and goals were being met? Budget  How much money will be used?
