Tahmid is a 26-year-old single man currently staying in a hostel for homeless people in Central London. He has presented himself to Lewisham Hospital Accident and Emergency department, where he has been seen by the Psychiatric Liaison Team and admitted to an acute inpatient ward for assessment. He is a voluntary patient and is willing to be in hospital. On the ward he is dishevelled and dirty in appearance and complaining of ‘very bad’ headaches. He is constantly carrying on a conversation even when alone and appears to be responding to voices. Assignment: Management of Risk in Mental Health and Learning Disability Services Tahmid looks very frightened and intermittently looks up to the ceiling and calls out for help. His speech changes from English to another language. You are asked to assist with the initial nursing assessment of Tahmid. He tells you that he can’t return to the hostel as everybody there is out to get him. He says that the other hostel residents are loud, and often get drunk and take drugs. He is unable to find a safe and quiet space to relax or to say his daily prayers. Tahmid also says that he has been in a ‘mental hospital’ for two weeks about a year ago. He said he was discharged because the staff were unable to help him either with his headaches, or ‘the others’ who are angry with him.He is reluctant to give any information about his background. He says that he lost contact with his family some months ago. He is originally from the North of England and says that he has a married sister living there with her family. Describe the main points you would want to cover in an assessment with Tahmid , paying attention to any risk factors she may be presenting with. Discuss the specific nursing interventions that will be required to meet the needs identified in the assessment. Think about the potential barriers and benefits of your role as a student mental health nurse, and reflect on developing a therapeutic relationship with Tahmid . Please read the following case scenario and answer both questions at the end . Joseph is a 29 year old man of mixed Nigerian and British heritage who has just been admitted informally onto an acute ward. He had been accompanied to the Mental Health Unit by his Care Coordinator who had become concerned about his risk of self-harm. Joseph disclosed some suicidal ideation earlier that day, and had begun cutting both his forearms. He also stated that he had begun to drink large amounts of strong cider again after a lengthy period of abstinence. On admission to the ward, Joseph presents as being quiet, withdrawn and occasionally tearful. His clothes are worn and dirty and he seems under-weight; his trousers are loose despite his belt being fastened on the tightest notch. In addition, Joseph has several teeth missing and is complaining of toothache. • Once on ward, Joseph informs staff that he was last admitted to this ward about 3 years ago. On checking his notes, you read that he was sexually abused as a child by a member of his family. In addition he has not been in contact with either his mother or step-father since moving away from their home in a socially deprived housing estate in a town on the east coast of England 10 years ago. Before his arrival on the ward, Joseph told his Care Co-ordinator that he couldn’t stop thinking about the ‘filthy pig’ who abused him sexually as a child, after seeing another high profile case of child sexual abuse on the television recently. He still blames his mother and step-father for what happened to him. • Joseph has recently moved into his own bed-sit accommodation nearby, after waiting several years in temporary housing for such an offer to be made to him. He is concerned that being in hospital will affect the payment of his state benefits, including housing benefit. 1.Describe the way you would organise the interview and how you would go about engaging with Joseph. Describe the main points you would want to cover in an assessment with Joseph, paying attention to any risk factors she may be presenting with. Discuss the specific nursing interventions that will be required to meet the needs identified in the assessment. Joseph Please read the following case scenario and answer both questions at the end . Farid is a 43-year-old man of Iranian heritage who has just been admitted informally to the acute admissions ward of a local mental health unit. He was accompanied to the unit by staff from a hostel for homeless people where he has been living for the past 18 months, as he had told them that he wanted to kill himself. Once on the ward, Farid is seen to be limping markedly and talking to himself in a language other than English. He is able to stop and converse in English for a few minutes, and informs them that he has nightmares where he hears the voice of his wife calling for him to help her. The hostel staff explain that Farid is a former car mechanic who was granted refugee status in the U.K. five years ago, following an escape from Iran where he was imprisoned, beaten and tortured. Since his arrival in the U.K., Farid has not managed to contact his wife who has also been imprisoned, and admits that he fears for her life. He has no family members in the U.K. His use of alcohol was reported by the Hostel staff to have increased significantly over the past two weeks. He told them that he was drinking more to stop the nightmares. You have been asked to assist in the process of admitting Farid onto the ward. Answer both of the following questions: 1 . Describe how you would go about setting up an interview for Farid.
