Option #1: Developing Service Plan Goals for a Member of a Special Population
The Portfolio Project demonstrates your ability to apply knowledge of the case management experience in human services by synthesizing the information from the readings, class work, scholarly sources, and students work and life experiences. The purpose of this project is to integrate human service needs with evidence-based interventions, and to apply both to a fictitious human service setting and population.
In this assignment, you are asked to complete a service plan, using the service plan template provided (located under this assignment in Module 8), for a client with multiple needs.
Directions:
Select one of the following clients:
Client A: Jerry, a 29-year-old African American male. Jerry presents with difficulty finding a job. Jerry has some developmental delays and a learning disorder. He is dyslexic. He recently lost his best friend, Roger, in a car accident. Jerry has been using alcohol daily to cope with his feelings of grief and loss. Jerry does not have a car and lives with his mom who often provides transportation to his doctor visits and job interviews. Jerrys mother believes he is being discriminated against by prospective employers because of his race, disability, and past legal involvement.
Client B: Kim is a 17-year-old Hispanic female who identifies as lesbian. She was recently kicked out of her house after her mom found a love letter she had written to a female schoolmate. Kims mom is a devout Southern Baptist who believes that homosexuality is a sin. Kims fathers whereabouts are unknown. He left Kims mom when Kim was an infant. She never knew him. She has been anxious, cant sleep, and stays with friends or sleeps in an alley near her high school. She has been taking showers at a local truck stop and walking to school. She loves school and wants to get a graduate degree in veterinary medicine, but she has no place to call home, no job, and is running out of spending money.
- Thoroughly identify at least four case management needs of the selected client in your service plan.
- Develop a list of strategies and interventions based in research to treat the case management needs of this client. Include a summary of at least three different interventions. Incorporate at least one scholarly journal article to provide support for your descriptions. (Hint: The CSU-Global Library is a great place to find your resource!)
- Distinguish between direct interventions and care coordination/advocacy that will be used in the treatment of this case management client.
- Identify and distinguish among the distinct roles of the case manager and client in your service plan.
- Address cultural considerations in treating the population in need.
- Describe and address ethical considerations applied in the case management treatment of this population.
Additional Requirements:
Discuss and cite a total of at least three scholarly sources (including the citation requirement noticed above in #3), other than assigned readings for this course, to support your analysis and positions. The CSU-Global Library is a good place to find scholarly sources. For any references used, include a list at the end of your service plan, as a separate reference page. Cite your sources in APA format, following the guidance provided in the CSU-Global Guide to Writing and APA (Links to an external site.).
Option #2: Documenting a Case Management Scenario

She loves school and wants to get a graduate degree in veterinary medicine, but she has no place to call home, no job, and is running out of spending money. One of Kims service plan goals is to reduce her feelings of anxiety. She has been seeing a counselor for two weeks and began taking Prozac to treat her anxiety and depression. She is also working to find part-time employment at a convenience store.
- You have just met for a one-hour, face-to-face visit with your selected client and now you must document; use the Case Management Progress Note Template to track the clients progress and document the services you provided.
- Thoroughly document the service encounter you have with your client and the interventions you provide. Incorporate at least one scholarly journal article to provide support your interventions. (Hint: The CSU-Global Library is a great place to find your resource!)
- Include additional interventions in the Next Steps section of the progress note. Cite the research to support additional interventions.
- Distinguish between direct interventions and care coordination/advocacy that will be used in the treatment of this case management client.

Part I: Overcoming Bias
In order to be an effective early childhood educator and anti-bias advocate, it is necessary, first and foremost, to recognize and challenge ones own biases. While this may not seem difficult on a surface level, there are, in fact, various strategies and practices for recognizing the overt and more subtle or covert biases one may possess.
In two to three pages, address the following:
- Describe yourself, including any relevant information about your own social identity and culture. Explain how your own identity and/or culture might influence your professional practice. If applicable, share your own experiences with privilege, oppression, and/or bias.
- Identify three strategies that you will use in order to explore and overcome your own biases.
- Explain why you chose these strategies and how exploring and overcoming your own biases can positively impact the children and families with whom you work.
Part II: Addressing Bias and Promoting Equity in Early Childhood Classrooms
Bias and discrimination affect people from all walks of life. In the early childhood field, bias and discrimination are especially troublesome, as they can have long-term impacts that can continue to affect children through adulthood. Read Document #1: Madison Place Preschool Scenario prior to beginning Part II.
In three to four pages, complete the following:
- Describe four strategies that you will use in order to address bias and promote equity among the young children in your classroom.
- Explain why you chose these strategies, and how they will help address bias and promote equity.
- Describe how you will implement these strategies to advocate for positive social change on behalf of the children in your classroom.
- Describe one way you will partner with families and/or the community in your advocacy efforts.
Part III: Access, Equity, and Promoting Change in Early Childhood Organizations
While advocating for access, equity, and change in early childhood classrooms is an important first step, early childhood settings support this advocacy effort on an organizational level.
In two to three pages, complete the following:
- Describe how you will advocate for anti-bias curriculum in your early childhood setting or one in which you might work.
- Explain why anti-bias curriculum should be adopted in in this setting.
- Explain why access and equity at the organizational level are important to developing early childhood programs that help promote the growth and development of children, families, and communities.
- Include a description of at least one organization with whom you might partner to promote access, equity, and social change for young children and families in your early childhood setting or one in which you might work, and explain why you have chosen this organization.
Part IV: The Positive Impacts of Promoting Access, Equity, and Change
In two pages, describe the significant and lasting impacts that access and equity can have on children, families, communities, and a global society.
Specifically, your response should include the following:
- An explanation of the benefits of advocating for access, equity, and change on childrens healthy development and learning.
- An explanation of how advocating for access and equity in early childhood classrooms and programs can benefit children, families, and communities, and provide a specific example.
- A recommendation for developmentally appropriate learning experiences for the following, with an explanation of how these recommendations reflect the principles of developmentally appropriate practice.
- Three families arrive an hour after preschool starts on a regular basis
- Dual-language learners, most of whom are less fluent in English than they are in their primary languages
- Three children in your classroom seem to be behind their peers developmentally. After meeting with the families, you discover that they live in poverty and some adult members of the family do not have high school diplomas
