Module 5 discussion
Discussions A/B
Complete the discussion in Group A or B as assigned by the instructor and then respond to the alternate group. All students are required to review all posts for general knowledge. Keep in mind that you will earn points for your initial discussion posts, as well as your responses. If you do not posts responses, you cannot earn full credit. Refer to the Discussion Rubric on how points are earned.
DISCUSSION A
Group A will complete the initial post. Group B will respond. You are called to the room and find Michael 2 hours old has been very irritable and spitting up large amounts of formula. He is very jittery. You do not know the maternal history before coming into the room so you advise the mother that you would like to return the baby to the nursery for an assessment and monitoring. In the medical record you focus on prenatal history. You witness the baby having some significant tremors. What would you suspect? Michaels VS are: T (axillary) – 36.0, RR- 70, HR- 166, he is very alert, irritable, and does not console easily. Is moving constantly, and sucking very vigorous on the pacifier you provide for comfort. You place the baby under the warmer and obtain what labs?
It is determined that this baby is going through opioid withdrawal. Neonatal Abstinence Syndrome requires close observation and scoring. Methadone may be used. Neonatal Abstinence Syndrome Shows both the signs of withdrawal, scoring and the nursing care of a baby with Neonatal Abstinence Syndrome.
Group B Response
What is your feeling and knowledge regarding breastfeeding this baby?
DISCUSSION B
Group B will complete the initial post. Group A will respond. Baby Leah had a very traumatic vaginal birth. After a vacuum extraction for fetal heart rate tracing changes and a resultant shoulder dystocia, Leah has a mild Erb’s palsy, some facial and truncal bruising. Pediatrician has ordered physical therapy for the palsy and the doctor expects routine care to be provided to her. In addition, do not forget the education you must give to the new parents regarding this injury. Due to the bruising there is an increase in bilirubin levels and some behavioral changes occur with Leah. Please describe what is different and what will you discuss with the family to ensure safety? The bilirubin level requires phototherapy. What precautions must you take? What is essential to manage and treat the elevated bilirubin in addition to phototherapy?
Group A Response Phototherapy is discontinued at the hospital, but at home the parents are encouraged to provide some ‘routine’ therapy for Hyperbilirubinemia. What would you suggest? Do not forget routine discharge instruction for this parent.

Assignment: Political and Sociocultural Influences

Assignment: Political and Sociocultural Influences
Assignment: Political and Sociocultural Influences
Week 1 discussion
Discussion: Political, Social, and Sociocultural Influences on Women’s Health
As an advanced practice nurse, you must remain current on health issues that commonly impact women such as birth control, abortion, family planning, the human papillomavirus (HPV) vaccine, and human immunodeficiency virus (HIV). Many of these women’s health issues are heavily influenced by political, social, and sociocultural factors. These influences might not only affect a woman’s ability or desire to receive care, but also a provider’s ability or willingness to offer care. How might political, social, and sociocultural factors influence your personal perceptions of these women’s health issues?
To prepare:
Review this week’s media presentation, as well as Chapter 1 of the Schuiling and Likis text.
Select and research one of the following women’s health issues: birth control, abortion, family planning, human papillomavirus (HPV) vaccine, human immunodeficiency virus (HIV) in women, or another issue approved by the course Instructor.
Consider the impact of political, social, and sociocultural factors on the women’s health issue you selected.
Reflect on how the personal perceptions of providers might influence their ability or willingness to care for women in relation to this issue.
Week 2 discussion
Discussion: Guidelines on Screening Procedures
The goal of health promotion and education is to help prevent disease in patients. However, this is not always successful, and disease it not always preventable. With disease, early detection is ideal, often making screening procedures a routine part of clinical care. Unfortunately, many patients do not utilize these health services. This can be attributed to lack of awareness or access to care, financial concerns, or even levels of comfort with health care providers. In your role as the advanced practice nurse, you must be aware of potential obstacles for patients and implement strategies to ensure patients receive necessary screenings. Although a variety of screening procedures are recommended for women at various stages of life, not all screenings are appropriate for all patients. Understanding the strengths and limitations of each screening, as well as current guidelines for use is essential to effectively facilitate patient care. For this Discussion, the course Instructor will assign a specific topic for you to research.
To prepare:
Review this week’s media presentation, as well as Chapters 6 and 8 of the Tharpe et al. text and the U.S. Department of Health and Human Services article in the Learning Resources.
Research guidelines on screening procedures for the topic assigned to you by the course Instructor (e.g., guidelines on screening for domestic violence, safety, nutrition, osteoporosis, heart disease, mental health, eating disorders, thyroid disease, pap smear, mammogram, cancer, and sexually transmitted infections). Note: The course Instructor will assign a topic to you by Day 1 of this week.
Reflect on strengths and limitations of the screening guidelines.
Consider how the guidelines might support your clinical decision making.
Week 3 discussion
Discussion: Contraception Treatments
Considering the potentially negative consequences of unintended pregnancy for a woman’s health and well-being, effective contraceptive treatments are an important part of gynecologic care. There are a variety of contraceptive treatment methods available for women, including hormonal, barrier, and fertility awareness options. Each method has its own strengths and limitations, and each patient often has individual factors that might also impact appropriateness of use. In your role as the advanced practice nurse, it is important to keep in mind that while you may make contraceptive recommendations to patients, contraceptive selection is a joint decision between the patient and the provider. For this Discussion, consider which contraceptive treatments would be most appropriate for the patients in the following three case studies:
Case Study 1:
A 23-year-old Caucasian female presents with concerns about mood swings around the time of her menses. She believes she has PMS and wants to know if there is medication to control it.
Case Study 2:
A 25-year-old Latina female presents with menstrual cramping that has been getting worse over time. She has never been pregnant and she has one male sex partner. Her gynecologic exam is normal.
Case Study 3:
A 33-year-old Caucasian female is being seen in clinic for contraception. She is using birth control pills, but forgets to take them because her work schedule changes every week. She has been married for 14 years and has two children. She is looking for an effective method that will be easy to remember. She has a history of chronic headaches and hypertension during pregnancy. She has never been treated for a sexually transmitted infection and is in a mutually monogamous relationship. Family history is significant for an aunt with breast cancer. She smokes half a pack of cigarettes per day. She is 5 ft. 8 in. and 215 lbs. Her vital signs are: BP 120/78, p 72, reg.
To prepare:
Review Chapter 12 of the Schuiling and Likis text and the Dragoman et al. article in this week’s Learning Resources
Select one of the three provided case studies. Reflect on the patient information.
Consider an appropriate contraception treatment for the patient case study you selected.
Think about how you might facilitate the selection of contraception treatments with patients who do not agree with your recommendations

Abstract:
Most existing health-related quality of life research concerns the impact of disease on patients. However, in several medical specialties including dermatology, oncology, and physical and mental disability, studies have been carried out investigating the impact of disease on the lives of families of patients. The aim of this paper is to review the literature which relates to the impact of disease on family members of patients. The OVIDSP Medline was selected as the primary database, Searches were limited to sources published in English. 158 papers were identified for review. The definition of “family” varied across the literature, and a broad definition was accepted in this review. This review shows that a wide variety of aspects of family members’ lives can be affected, including emotional, financial, family relationships, education and work, leisure time, and social activities. Many of these themes are linked to one another, with themes including financial impact and social impact being linked to emotional impact. Some positive aspects were also identified from the literature, including family relationships growing stronger. Several instruments exist to measure the impact of illness on the family, and most are disease or specialty- specific. The impact of disease on families of patients is often unrecognised and underestimated. Taking into account the quality of life of families as well as patients can offer the clinician a unique insight into issues such as family relationships and the effect of treatment decisions on the patient’s close social group of partner and family. Assignment:Global Nursing Opportunities

Assignment:Global Nursing Opportunities
Assignment:Global Nursing Opportunities
The Course Outcome covered this week is CO5.
CO5: Discuss the implications of global nursing for healthcare delivery systems. (PO7)
Reflect on the TCN Self-Assessment you completed during Week 2, and determine if your score changed.
Perform an Internet search, and locate one Global Nursing Opportunity sponsored by a professional organization that utilizes nurses as either paid employees or volunteers. Name the organization, and include a working link. In your own words, share the organization’s mission.
Share two contributions you could make to the organization (patients served) based on your current experiences and future career aspirations based on your learning from this course.
Identify personal opportunities for improvement in providing culturally competent care.
Do you want to make a difference in global health? I’m happy to tell you that there are many opportunities available to serve internationally, especially as the world becomes more like a global village. As a professional nurse, I have found global service rewarding and fulfilling. As I write this column, images come to my mind of sitting in a Palestinian refugee camp in Jordan with an expectant mother suffering from thalassemia, or listening to a Mayan mother in the highlands of Guatemala in a stick-walled, corrugated roofed home talk about life with her newborn and seven other children.
As maternal/child nurses, we are all concerned about vulnerable and underserved populations and about how our healthcare colleagues with limited resources and opportunities do their work everyday. If you want to make a difference in global health, there are multiple global health opportunities for you; it’s a matter of finding a match for your clinical skills, academic credentials, personal and family limitations, time commitment, and state of health. For example,
* Seeking employment through such agencies as the American Embassy and Department of State, or the World Health Organization. The Global Health Council website has a listing of such employment opportunities (http://www.globalhealth.org).
* Providing refugee relief by working as an international refugee worker (http://www.refugeerelief.org).
* Providing disaster relief services through agencies such as the International Committee of the Red Cross (http://www.icrc.org).
* Providing cultural experiences for nursing students globally. A number of schools of nursing have international programs, and are often seeking mentors to accompany students to a variety of international settings, particularly in developing countries.
* Providing clinical mentorship in an international setting. Increasingly, the focus is on building local clinical skills through teaching programs such as neonatal resuscitation education programs that require a commitment to “training the trainer” so you can share your expertise with local nurses, midwives and physicians. Cross Cultural Solutions volunteers can match your expertise with global needs (http://www.crossculturalsolutions.org).
* Working with private, nonprofit secular or faith-based organizations such as Ixmucane, One Heart, Empowering Lives International, Doctors Without Borders, or Nazarene Compassionate Ministries. Some opportunities are as short as 10 days, and others require a commitment of at least 6 months.
* Sharing your expertise in policy and program consultation through organizations such as United States Agency for International Development (USAID).
* Partnering with academic colleagues overseas by offering to be a visiting professor, an educational consultant, or applying to become a Fulbright Scholar (http://www.cies.org/about_fulb.htm).
Whether you are accepted by an international educational and relief organization depends on your previous experience in global settings or working with vulnerable populations such as refugees and immigrants, any work you’ve done in disasters or emergencies, skills in working independently in remote circumstances, having linguistic skills, having clinical skills in maternal/child health, and being in good health. It is essential to work through established organizations to do global work rather than “dropping in,” on a disaster site-which actually could end up contributing to the chaos rather than helping!!
I encourage you to explore global opportunities (Osborn & Ohmans, 2005). Etherington (2009, p. 10), who has worked in 13 countries primarily in acute disaster and conflict settings, has said, “Going global has its risks and hardships, but they are usually outnumbered by rewards, wonders, and a profound sense of purpose. You have much to offer. Take the responsibility of a global mission seriously and savor it as one of the most important ventures of your life-as a nurse and as a citizen of the world.”

Challenges In Developing Care Plan

Challenges In Developing Care Plan
Challenges In Developing Care Plan
Week 1 Project
Introduction
While treating the chronically ill, a major challenge is developing a plan of care that addresses the specific needs of a patient and a caregiver. You need to be in close touch with patients and their support group, family and peers, to come up with an ideal plan.
In this course project, you are going to develop a plan of care for a chronic illness group of your choice using the Healthy People 2020 Topics. While executing the tasks of this project, remember that while you need to give a general overview of the biomedical considerations of the case, the focus should always be on the psychosocial elements. Your perspective in this care plan should be the patient’s goals rather than those of the medical team.
Each week, you will complete a part of this project. You will submit a final completed project in Week 5. Be sure to take the time to carefully complete each of the weekly assignments and then put it all together in Week 5.
Ensure that you save a copy of this course project after you have submitted it at the end of this course. You are expected to resubmit this project along with the other course projects at the end of the Registered Nurse (RN) to Bachelor of Science in Nursing (BSN) program. File Transfer Protocol (FTP) details will be provided in the Capstone course.
Identifying a Group
Identify an area of chronic illness of specific interest to you and that is represented as a Healthy People 2020 topic (healthypeople.gov). Explain your choice and your interest in it. Prepare a questionnaire utilizing your knowledge of health and illness, with the aim of acquiring all information you need from patients to prepare a plan of care for the specific illness group. Submit your information in a 4- to 5-page Microsoft Word document. In addition to your questionnaire, be sure to include the following details in your paper.
Chronic Illness of interest
Morbidity and comorbidity of the disease
Impact of the chronic illness and patient morbidity and the affect to overall health of nation
Healthy People 2020 goals and objectives for specific illness group
Support your responses with examples.
On a separate references page, cite all sources using APA format.
Use this APA Citation Helper as a convenient reference for properly citing resources.

Common Illnesses and the Use of CAM

Common Illnesses and the Use of CAM
Common Illnesses and the Use of CAM
The Course Outcome covered this week is CO1.
CO1: Identify theories, concepts, and beliefs related to transcultural nursing. (PO1)
We encounter patients after they have explored various avenues of self-help. At times, these avenues involve the use of complementary and alternative medicine (CAM). Our readings this week are very helpful.
Select a common (or not so common) illness or condition that affects a specific ethnic group.
Identify two (2) CAM therapies that patients are likely to try before seeking (or in addition to) allopathic, osteopathic, or ayurvedic medical intervention for the illness or condition.
Discuss the effectiveness of each therapy you identified.
Note: Do not discuss an illness or condition about which a classmate has already posted. The diversity will lead to a more robust discussion. There are many examples you may discuss, including sickle-cell disease in African Americans, constipation in the elderly people, or the use of Reiki for mental and emotional healing by Japanese people. Think of others from your practice settings or prelicensure experiences.
bjectives. The purpose of our study was to determine the prevalence of complementary and alternative medicine (CAM) use and its clinical and psycho-social correlates, including perceived satisfaction with care and cultural health beliefs.
Methods. A cross-sectional study was carried out in public sector primary care clinics in Singapore using a random sample of 488 adult patients with chronic diseases. The measures were CAM use, satisfaction with care and traditional health beliefs.
Results. The 1 year prevalence of CAM use was 22.7%. In univariate analyses, factors associated with CAM use included: middle age, arthritis, musculoskeletal disorders and stroke, multiple conditions, poor perceived health, family use of CAM, recommendation by close social contacts, strong adherence to traditional health beliefs and perceived satisfaction with care. Patients who were dissatisfied/very dissatisfied with the cost of treatment [odds ratio (OR) = 1.79, 95% confidence interval (CI) 1.15–2.82] and waiting time (OR = 1.96, 95% CI 1.20–3.19) were more likely to use CAM. Patients who were very satisfied with the benefit from treatment were much less likely to use CAM (OR = 0.49, 95% CI 0.29–0.83). Satisfaction with doctor–patient interaction was not associated with CAM use. Being ‘very satisfied’ on overall care satisfaction was significantly associated with much less CAM use (OR = 0.30, 95% CI 0.14–0.68). Multivariate analyses confirmed that CAM use was significantly and independently predicted by the ‘chronic disease triad’ (arthritis/musculoskeletal disorders/stroke) (OR = 4.08, 95% CI 2.45–6.83), overall satisfaction with care (OR = 0.32, 95% CI 0.14–0.74) and strong adherence to traditional health beliefs (OR = 1.88, 95% CI 1.07–3.31).
Conclusion. CAM use in Asian patients is prevalent and associated with the ‘chronic disease triad’ (of arthritis, musculoskeletal disorders and stroke), satisfaction with care and cultural beliefs. In particular, CAM use is not associated with the quality of doctor–patient interaction.

Assignment:Planning Care for an Elderly Person
Assignment:Planning Care for an Elderly Person
Week 2 Project
Assessing and Planning Care for an Elderly Person
In order to gain an insight into the world of elder adults, it is important to understand how they view themselves and the values they hold. Additionally it is important to assess and determine his/her needs and establish appropriate interventions for this individual.
By the due date assigned, choose an older adult to interview. This cannot be a patient in your clinical setting. You can use a friend, family member, or co-worker. The older adult should be 65 years or older. Use the format provided to record the responses. A list of questions is available for you to start with. Include 2–3 questions of your own to get a complete picture of the older adult. Summarize your findings and also contrast the responses with findings in your readings and other current literature.
Download a patient questionnaire. This form should be used as an example.
After gaining permission, conduct a physical and mental functional assessment of the older adult you have chosen. Review your readings for the process of functional assessment.
Make use of the tools discussed this week to complete a comprehensive assessment of your patient. Search the Internet for resources on these tools.
Tinetti Balance and Gait Evaluation
Katz Index of Activities of Daily Living
Assessment of Home Safety
The Barthel Index
Make sure the older adult is clearly identified on the tools. Do not include their name, but do include professional or other designation, and age. Your name should also be identified on the tool. (This should be a part of your Appendix.)
Compare and contrast the age-related changes of the older person you interviewed and assessed with those identified in this week’s reading assignment. During this data analysis process provide at least 4–6 preliminary issues that you have identified. Identify three alterations in health that you would propose and describe them. Identify a minimum of three comprehensive interventions for each problem.
Make sure that you integrate personal cultural awareness and cultural competency.
Paper should be 5–6 pages, not including the questionnaire or tools used. These should be attached as an Appendix. Remember to use headings to identify the different sections in your paper.
On a separate references page, cite all sources using APA format.

Assignment:Organizational Culture and Readiness

Assignment:Organizational Culture and Readiness
Assignment:Organizational Culture and Readiness
Week 2 assignment
Evidence-Based Practice Proposal – Section A: Organizational Culture and Readiness Assessment
Before making a case for an evidence-based project, it is essential to understand the culture of the organization in order to begin assessing its readiness for EBP implementation. Select an appropriate organizational culture survey tool and use this instrument to assess the organization’s readiness.
Develop an analysis of 250 words from the results, addressing your organization’s readiness level, possible project barriers and facilitators, as well as how to integrate clinical inquiry.
Make sure to include the rationale for the survey categories scores that were significantly high and low, incorporating details and/or examples. Also explain how to integrate clinical inquiry into the organization, providing strategies that strengthen the organizations weaker areas.
Submit a summary of your results. The actual survey results do not need to be included.
Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.
This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.
You are required to submit this assignment to Turnitin. Please refer to the directions in the Student Success Center.
Upon receiving feedback from the instructor, revise “Section A: Organizational Culture and Readiness Assessment” for your final paper submission. This will be a continuous process throughout the course for each section.
Week 2 assignment
Evidence-Based Practice Proposal – Section B: Problem Description
Write a paper of 500-750 words (not including the title page and reference page) on your proposed problem description for your EBP project. The paper should address the following:
Describe the background of the problem. Tell the story of the issue and why it deserves attention.
Identify the stakeholders/change agents. Who, or what organizations, are concerned, may benefit from, or are affected by this proposal. List the interested parties, patients, students, agencies, Joint Commission, etc.
Use the feedback from the Topic 2 main forum post and refine your PICOT question. Make sure that the question fits with your graduate degree specialization.
State the purpose and project objectives in specific, realistic, and measurable terms. The objective should address what is to be gained. This is a restatement of the question, providing focus. Measurements need to be taken before and after the evidence-based practice is introduced to identify the expected changes.
5) Provide supportive rationale that the problem or issue is an important one for nursing to resolve using relevant professional literature sources.
Develop an initial reference list to assure that there is adequate literature to support your evidence-based practice project. Follow the “Steps to an Efficient Search to Answer a Clinical Question” box in chapter 3 of the textbook. Use “NUR-699 Search Method Example” to assist you.
7) The majority of references should be research articles. However, national sources such as Centers for Disease Control and Prevention (CDC), National Center for Health Statistics (NCHS), Department of Health and Human Resources (HHS), or the Agency for Healthcare Research and Quality (AHRQ) and others may be used when you are gathering statistics to provide the rationale for the problem.
Once you get into the literature, you may find there is very little research to support your topic and you will have to start all over again. Remember, in order for this to be an evidence-based project, you must have enough evidence to introduce this as a practice change. If you find that you do not have enough supporting evidence to change a practice, then further research would need to be conducted.
Upon receiving feedback from the instructor, refine “Section B: Problem Description” for your final submission. This will be a continuous process throughout the course for each section.
Week 3 assignment
Evidence-Based Practice Proposal – Section C: Literature Support
To begin, work through the reference list that was created in the “Section B: Problem Description” assignment in Topic 2. Appraise each resource using the “Rapid Critical Appraisal Checklists,” available in the textbook appendix. The specific checklist you use will be determined by the type of evidence within the resource.
Develop a research table to organize and summarize the research studies. Using a summary table allows you to be more concise in your narrative description. Only research studies used to support your intervention are summarized in this table. Refer to the “Evaluation Table Template,” available in the textbook appendix. Use the “Evaluation Table Template” as an adaptable template.
Write a narrative of 750-1,000 words (not including the title page and references) that presents the research support for the projects problem and proposed solution. Make sure to do the following:
Include a description of the search method (e.g., databases, keywords, criteria for inclusion and exclusion, and number of studies that fit your criteria).
Summarize all of the research studies used as evidence. The essential components of each study need to be described so that readers can evaluate its scientific merit, including study strengths and limitations.
Incorporate a description of the validity of the internal and external research.
It is essential to make sure that the research support for the proposed solution is sufficient, compelling, relevant, and from peer-reviewed professional journal articles
Although you will not be submitting the checklist information or the evaluation table you design in Topic 3 with the narrative, the checklist information and evaluation table should be placed in the appendices for the final paper.

Week 4 assignment
Evidence-Based Practice Proposal – Section D: Solution Description
Write a paper of 500-750 words (not including the title page and reference page) paper for your proposed evidence-based practice project solution. Address the following criteria:
Proposed Solution: (a) Describe the proposed solution (or intervention) for the problem and the way(s) in which it is consistent with current evidence. Heavily reference and provide substantial evidence for your solution or intervention. (b) Consider if the intervention may be unrealistic in your setting, too costly, or there is a lack of appropriate training available to deliver the intervention. If the intervention is unrealistic, you may need to go back and make changes to your PICOT before continuing.
Organization Culture: Explain the way(s) in which the proposed solution is consistent with the organization or community culture and resources.
Expected Outcomes: Explain the expected outcomes of the project. The outcomes should flow from the PICOT.
Method to Achieve Outcomes: Develop an outline of how the outcomes will be achieved. List any specific barriers that will need to be assessed and eliminated. Make sure to mention any assumptions or limitations which may need to be addressed.
Outcome Impact: Describe the impact the outcomes will have on one or all of the following indicators: quality care improvement, patient-centered quality care, efficiency of processes, environmental changes, and/or professional expertise.
