Week 6 discussion
DQ1 Although you are not implementing your project at this point and conducting studies with human participants, IRB is an important step in research State the required components one should look for in a project to determine if IRB submission is needed Discuss an example of a research study found in one of your literature review articles that needed IRB approval Specifically, describe why IRB approval was needed in this instance
DQ2 Visit the Yale Center for Bioethics Cases in Research Ethics website Choose one of the three cases below to read and respond to in this weeks discussion Be sure to read the background of the study, the ethical and regulatory issues raised by the IRB, and the comments presented at the end of each case
Choose from one of these three cases:
Case 1: Payments to subjects who are substance abusers
Case 5: Reasonable right to privacy for patients accessing hospital services
Case 6: An ethnographic study of homeless adolescents
Address the following in your response:
Identify the key ethical issues that are questioned in this case and the category of risk: physical, psychological, social, or economic harm
Do you feel the level of questioning and concern by the IRB was appropriate? Were you surprised by any of the IRBs decisions? Explain
Identify a risk or red flag in this study that you may not have considered on your own prior to reading this study
Present a new perspective you gained about the IRB, the type of study you read about, the ethics involved, or human subject research processes
In your replies to peers, respond to at least one peer who chose a different study
Week 7 discussion
All research projects are based around variables, which are essentially the characteristics of an individual, group, or the environment that is of interest in a research study Variables can be straightforward and measurable (eg, gender, age, amounts, funding) Variables can also be complex (eg, socioeconomic status of a group, attitudes toward a topic, aspects of a system) Now that your research topic has been identified and you have reviewed the overall literature and constructed your framework, you should identify the key variables of interest In the discussion board this week, free write a list of variables that revolve around and within your topic of study Are there variables that are of particular interest or focus in your study? Can you narrow your study down based on your list of variables? Is there anything you need to do to narrow your scope of research and your research questions?
Week 8 discussion
In session B of the Culminating Experience, you will be diving into the methodology you would use if you were to conduct your research The methodology section describes the specific procedures or techniques used to identify, select, and analyze data in order to understand your identified research problem The methodology section of a research paper answers two main points: how the data was collected and how it was analyzed This week, post a preliminary outline about your proposed methodology for your research problem If you were to move to full implementation, how would you collect and analyze the data? Would your study be qualitative or quantitative and why? In week 1 of Session B, you will submit your full methodology draft for your proposal, so please use this forum to help formulate your ideas
Week 2 Problem Identification, Research Questions, and Description
This week, submit a 12 page problem identification and description of the project you have chosen for your culminating project that includes:
A cover page
A title and abstract: 150200 words describing the project
A problem statement, problem identification, or purpose of the project: Provide a concise statement of the problem, need, or questions being investigated Give a clear and explicit statement of the problem, including the target population and background information relative to the specific problem Include 12 research questions that you will explore within your research
A preliminary review of the literature: List, reference, and summarize 34 peer-reviewed sources that support your identified project and the need for change, the issue at hand, or the need for further research Be sure that the data supports the need for investigation or change and that your problem is not one that has already been widely solved
You will have opportunities to refine your problem statements and research questions throughout this course Be sure to review your instructors feedback
Week 4 Integrative Literature Review
Write a literature review of your topic using peer-reviewed articles and books, as well as non-research literature such as evidence-based guidelines, toolkits, and standardized procedures Identify and cite all sources of data according to APA guidelines
In at least 5 pages:
Critique the most current research to support your problem; this research will help drive the focus of your project
Summarize the key findings of the research and its relevancy to your project
Describe any gaps in knowledge that you found and the effects this may have on health informatics as it relates to your project topic The literature review should be a synthesis of how each article or study relates to your project
When writing your literature review, remember to include subtopics to your main topic and gather data on these areas as well
Your integrative literature review should be at least 5 pages in length, not including the cover or reference pages, and must contain a minimum of 10 scholarly articles published within the past 57 years
This literature is considered a draft and will need to be revised for integration into your final paper Please make sure to review your instructors feedback
Week 5 Theoretical or Conceptual Framework
This week, review your literature and look for gaps of knowledge in your topic What is missing? What questions do you still have that you can potentially solve in your research? Compose a 2- to 3-page paper that outlines the framework for your research project
Address the following:
Explain the problem and briefly summarize the research you have completed in your literature review
Define any gaps or relationships that are apparent and how you will address them in your project Be sure to reference back to your research questions and problems to ensure that the gaps correlate with your problem and questions
Demonstrate that the solution you propose is not based on your personal opinions or guesses, but are formed from the facts that you have read from the authors of the previous research
You may find that you are adding onto your references and your literature review based on new investigation and that is okay!
Explain your perspective and reference any nursing or informatics theoretical underpinnings if it supports your framework
Week 7 Predictions, Outcomes, and Variables
Develop your project predictions and desired outcomes portion of your paper using the following as starting points:
Describe the desired outcomes: Specifically, state the purpose, focus, and viewpoint of the project as well as its expected accomplishments While you may not be implementing your project, you should have a goal in mind that relates to solving your problem
A project goal should reference the projects benefits in terms of cost, time, and/or quality that address needs or changes, results, impacts, or consequences that the project has on people, programs, or institutions
Goals and objectives should be measurable, shared, and hypothetically agreed on by all key stakeholders They are directly linked to the concept of project success factors
What variables need to be considered and would you have control over them? For example, if you were to implement a department wide system change, would budget be an issue and how would you address that variable?
If you were to implement your study, how would you gather data? Would the focus be qualitative or quantitative?
What will your research provide to the community or to social change?
In the statistical sense, what is already known about your problem? Cite evidence from your literature review
Compose a 2- to 3-page paper that addresses the intended outcomes of your project

Benchmark Evidence-Based Practice Proposal Final Paper
Throughout this course you will be developing a formal, evidence-based practice proposal.
The proposal is the plan for an evidence-based practice project designed to address a problem, issue, or concern in the professional work setting. Although several types of evidence can be used to support a proposed solution, a sufficient and compelling base of support from valid research studies is required as the major component of that evidence. Proposals are submitted in a format suitable for obtaining formal approval in the work setting. Proposals will vary in length depending upon the problem or issue addressed; they can be between 3,500 and 5,000 words. The cover sheet, abstract, references page, and appendices are not included in the word limit.
Section headings and letters for each section component are required. Responses are addressed in narrative form in relation to that number. Evaluation of the proposal in all sections is based upon the extent to which the depth of content reflects graduate-level critical-thinking skills.
This project contains seven formal sections:
Section A: Organizational Culture and Readiness Assessment
Section B: Problem Description
Section C: Literature Support
Section D: Solution Description
Section E: Change Model
Section F: Implementation Plan
Section G: Evaluation of Process
Each section (A-G) will be submitted as separate assignments so your instructor can provide feedback (refer to applicable modules for further descriptions of each section).
The final paper will consist of the completed project (with revisions to all sections), title page, abstract, reference list, and appendices. Discussion:Diagnostic Tests in Adults and Children
Discussion:Diagnostic Tests in Adults and Children
Discussion:Diagnostic Tests in Adults and Children
When seeking to identify a patients health condition, advanced practice nurses can use a diverse selection of diagnostic tests and assessment tools; however, different factors affect the validity and reliability of the results produced by these tests or tools. Nurses must be aware of these factors in order to select the most appropriate test or tool and to accurately interpret the results.
Not only do these diagnostic tests affect adults, body measurements can provide a general picture of whether a child is receiving adequate nutrition or is at risk for health issues. These data, however, are just one aspect to be considered. Lifestyle, family history, and culture-among other factors-are also relevant. That said, gathering and communicating this information can be a delicate process.
In this Discussion, you will consider the validity and reliability of different assessment tools and diagnostic tests. You will explore issues such as sensitivity, specificity, and positive and negative predictive values. You will also consider examples of children with various weight issues. You will explore how you could effectively gather information and encourage parents and caregivers to be proactive about their childrens health and weight.
To prepare:
Review this weeks learning resources and consider factors that impact the validity and reliability of various assessment tools and diagnostic tests. We will also review examples of pediatric patients and their families as it relates to BMI.
This week you will be assigned one of the following DB by your instructor.
Select one of the following assessment tools or diagnostic tests to explore for the purposes of this Discussion:
Mammogram
Prostate-specific antigen (PSA) test
Body-mass index (BMI) using waist circumference for adults
Select one of the examples on which to focus for this Discussion. What health issues and risks may be relevant to the child you selected?
Overweight 5-year-old boy with overweight parents
5-year-old girl of normal weight with obese parents
Severely underweight 12-year-old girl with underweight parents
Search the Walden Library and credible sources for resources explaining the tool or test you selected. What is its purpose, how is it conducted, and what information does it gather?
What does the literature discuss regarding the validity, reliability, and are there any issues with sensitivity, specificity, predictive values, ethical dilemmas, and controversies related to the test or tool?
Based on the risks you identified, consider what further information you would need to gain a full understanding of the childs health. Think about how you could gather this information in a sensitive fashion.
Consider how you could encourage parents or caregivers to be proactive toward the childs health.
By Day 3
Post a description of how the assessment tool or diagnostic test you selected is used in health care. Based on your research, evaluate the test or the tools validity and reliability, and explain any issues with sensitivity, reliability, and predictive values. Include references in appropriate APA formatting.
OR
Post an explanation of the health issues and risks that are relevant to the child you selected. Describe additional information you would need in order to further assess his or her weight-related health. Taking into account the parents and caregivers potential sensitivities, list at least three specific questions you would ask about the child to gather more information. Provide at least two strategies you could employ to encourage the parents or caregivers to be proactive about their childs health and weight.
Note: For this Discussion, you are required to complete your initial post before you will be able to view and respond to your colleagues postings. Begin by clicking on the Post to Discussion Question link and then select Create Thread to complete your initial post. Remember, once you click on Submit, you cannot delete or edit your own posts, and you cannot post anonymously. Please check your post carefully before clicking on Submit!
Read a selection of your colleagues responses.
By Day 6
Respond to at least two of your colleagues who selected a different tool test, or child health example than you, using one or more of the following approaches:
Critique your colleagues evaluation of the validity and reliability of the tool or test selected.
Suggest alternative or additional tools or tests that should be considered when gathering information about specific conditions or symptoms.
Based on the risks you identified, consider what further information you would need to gain a full understanding of the childs health. Think about how you could gather this information in a sensitive fashion.
Consider how you could encourage parents or caregivers to be proactive toward the childs health.
Suggest additional health risks or issues that could be relevant to the child.
Critique your colleagues questions, and suggest how the parents or caregivers might interpret these questions. Provide alternate or additional questions.
Suggest an additional strategy for gathering patient information or promoting proactivity
Week 5 discussion
Discussion: Assessing the Ears, Nose, and Throat
Most ear, nose, and throat conditions that arise in non-critical care settings are minor in nature. However, subtle symptoms can sometimes escalate into life-threatening conditions that require prompt assessment and treatment. Nurses conducting assessments of the ears, nose, and throat must be able to identify the small differences between life-threatening conditions and benign ones. For instance, if a patient with a sore throat and a runny nose also has inflamed lymph nodes, the inflammation is probably due to the pathogen causing the sore throat rather than a case of throat cancer. With this knowledge and a sufficient patient health history, a nurse would not need to escalate the assessment to a biopsy or an MRI of the lymph nodes, but would probably perform a simple strep test.
In this Discussion, you consider case studies of abnormal findings from patients in a clinical setting. You determine what history should be collected from the patients, what physical exams and diagnostic tests should be conducted, and formulate a differential diagnosis with several possible conditions.
Note: By Day 1 of this week, your Instructor will have assigned you to one of the following specific case studies for this Discussion. Also, your Discussion post should be in the Episodic/Focused SOAP Note format, rather than the traditional narrative style Discussion posting format. Refer to Chapter 2 of the Sullivan text and the Episodic/Focused SOAP Template in the Week 5 Learning Resources for guidance. Remember that all Episodic/Focused SOAP notes have specific data included in every patient case.
Case 1: Nose Focused Exam
Richard is a 50-year-old male with nasal congestion, sneezing, rhinorrhea, and postnasal drainage. Richard has struggled with an itchy nose, eyes, palate, and ears for 5 days. As you check his ears and throat for redness and inflammation, you notice him touch his fingers to the bridge of his nose to press and rub there. He says hes taken Mucinex OTC the past two nights to help him breathe while he sleeps. When you ask if the Mucinex has helped at all, he sneers slightly and gestures that the improvement is only minimal. Richard is alert and oriented. He has pale, boggy nasal mucosa with clear thin secretions and enlarged nasal turbinates, which obstruct airway flow but his lungs are clear. His tonsils are not enlarged but his throat is mildly erythematous.
Case 2: Focused Throat Exam
Lily is a 20-year-old student at the local community college. When some of her friends and classmates told her about an outbreak of flu-like symptoms sweeping her campus over the past two weeks, Lily figured she shouldnt take her three-day sore throat lightly. Your clinic has treated a few cases similar to Lilys. All the patients reported decreased appetite, headaches, and pain with swallowing. As Lily recounts these symptoms to you, you notice that she has a runny nose and a slight hoarseness in her voice but doesnt sound congested.
Case 3: Focused Ear Exam
Martha brings her 11-year old grandson, James, to your clinic to have his right ear checked. He has complained to her about a mild earache for the past two days. His grandmother believes that he feels warm but did not verify this with a thermometer. James states that the pain was worse while he was falling asleep and that it was harder for him to hear. When you begin basic assessments, you notice that James has a prominent tan. When you ask him how hes been spending his summer, James responds that hes been spending a lot of time in the pool.
To prepare:
With regard to the case study you were assigned:
Review this weeks Learning Resources and consider the insights they provide.
Consider what history would be necessary to collect from the patient.
Consider what physical exams and diagnostic tests would be appropriate to gather more information about the patients condition. How would the results be used to make a diagnosis?
Identify at least 10 possible conditions that may be considered in a differential diagnosis for the patient.
Note: Before you submit your initial post, replace the subject line (Week 5 Discussion) with Review of Case Study ___, identifying the number of the case study you were assigned.

Directions
Refer to the examples in Chapter 4 of your textbook that discuss development of a genogram.
Download the required NR305_Family_Genetic_History_Form (Links to an external site.)Links to an external site.. You will document the adult participants family genetic history using this MS Word document.
Complete the family genetic history using the information that the adult participant is willing to share with you. The focus of this course is on the normal healthy individual so your paper does not need to contain much medical/nursing detail. Refer to your textbook or the Internet to learn what impact the familys health history may have on the adult participants personal state of wellness both now and in the future. This paper does not require APA formatting, but you are expected to write clearly and use proper grammar and spelling. Developing a pictorial genogram using symbols to identify certain relationships (e.g., divorced, sibling, deceased, etc.), may provide more insight, however, drawing may be difficult to accomplish with MS Word, therefore you are not expected to use symbols, lines, or other drawing elements. Instead, describe the relationships among the various people in the adult participants familys genetic history. Remember, the goal is not to learn how to draw with Word, but to gather information about the family and recognize its significance to the adult participant and that persons health.

This paper is expected to be no more than four pages in length (not including the title page and reference list).
Please use the categories listed below as the first level headings for each section of your paper (Exception: Instead of Introduction please use the title of your paper).
Content Guidelines
Using the information gathered in your Genetic Family History and Milestone 1 assignments, you will identify and research one modifiable risk factor. You will develop an evidence-based teaching plan that promotes health, with the aim of improving patient outcomes related to this modifiable risk factor.
Introduction (1 paragraph): The introduction should be interesting and capture the readers attention.
State one preventable disease your adult participant is at risk for developing (obesity, Type 2 Diabetes, etc.), based on the information you have gathered during this course.
Introduce one modifiable risk factor (diet, smoking, sedentary lifestyle, etc.) associated with the preventable disease you described.
Clearly state the purpose of your paper. The purpose statement should relate to developing a patient teaching plan.
Preventable Disease Overview (1-2 paragraphs): Please use course materials and scholarly sources to complete this section. Be sure to paraphrase and include citations as needed!
Describe briefly the preventable disease you identified in the introduction.
What are the signs and symptoms associated with this disease?
How is it diagnosed? Notable information from Health History and General Survey? Signs and symptoms? Risk assessments? Lab values?
What physical assessment findings would be associated with this disease?
How did you identify your adult participants risk for this disease?
Specific information gathered in the Genetic Family History?
Specific information gathered in the Milestone 1 assignment?
Evidence Based Intervention (1-2 paragraphs):
Choose and describe one evidence based intervention related to the modifiable risk factor you have chosen. (One that has been shown to be effective at reducing an individuals risk for developing the preventable disease.)
Describe this intervention in detail, and provide rationale for your intervention utilizing:
At least one scholarly peer-reviewed journal article. Go to the Chamberlain library at http:/library.chamberlain.edu (Links to an external site.)Links to an external site.. Search peer reviewed journals to find a scholarly article that discusses interventions associated with the modifiable risk factor you have chosen as the focus of your paper. Summarize what you learned from this article and cite the source in the text and reference page (Author, year). The article must be less than 10 years old. You may include and cite your textbook or other scholarly sources as well, but they do not count as the scholarly journal article.
You must summarize and analyze the information from your sources in your own words. Avoid direct quotations. If you do include a short quotation, be sure to cite it properly.
List short and long term goals you have for your participant related to this intervention.
Implementation: Teaching Plan (1-2 paragraphs):
Discuss in this section what methods you will use to teach your participant how to implement the intervention.
What is it that you are asking them to do? (For example, if your intervention is quit smoking, what specific steps will you recommend your participant take to achieve this goal?)
Describe any resources you would provide to the participant to help them understand and be successful (websites, community services, handouts, etc.).
Evaluation (1 paragraph):
Describe at least one method (weight, lab values, etc.) you would use to evaluate whether your intervention was effective.
How would you revise you plan if it proved to be unsuccessful?
Summary (1 paragraph):
Summarize the main points of your paper.
Reiterate what you hope to accomplish by implementing this intervention.
End with a concluding statement.
Reference Page:
The Reference Page should start on a new page (insert a page break).
All references should be cited within the body of the paper as (Author, year) and the full reference should be included in APA format on the reference page.
A URL link alone is not an adequate reference.
See the Chamberlain Guidelines for Writing Professional Papers in Course Resources for examples of properly formatted references.
Best Practices Tips for Writing a Great Paper!
Review the directions, guidelines, and grading rubric thoroughly.
Use the APA template provided.
Be clear. Write about one preventable disease, one modifiable risk factor, and one related intervention.
Proofread prior to final submission. Having another person review your work is often helpful to catch grammar and spelling errors. (Spell check is not perfect!)
Abide by the CCN academic integrity policy.
