Assignment: Treating Clients with With Bipolar Disorder
Assignment: Treating Clients with With Bipolar Disorder
Bipolar disorder is a unique disorder that causes shifts in mood and energy, which results in depression and mania for clients. Proper diagnosis of this disorder is often a challenge for two reasons: 1) clients often present as depressive or manic, but may have both; and 2) many symptoms of bipolar disorder are similar to other disorders. Misdiagnosis is common, making it essential for you to have a deep understanding of the disorders pathophysiology. For this Assignment, as you examine the client case study in this weeks Learning Resources, consider how you might assess and treat clients presenting with bipolar disorder.

Week 5 assignment
Assignment: Assessing and Treating Clients With Anxiety Disorders
Common symptoms of anxiety disorders include chest pains, shortness of breath, and other physical symptoms that may be mistaken for a heart attack or other physical ailment. These manifestations often prompt clients to seek care from their primary care providers or emergency departments. Once it is determined that there is no organic basis for these symptoms, clients are typically referred to a psychiatric mental health practitioner for anxiolytic therapy. For this Assignment, as you examine the client case study in this weeks Learning Resources, consider how you might assess and treat clients presenting with anxiety disorders.

Week 2 assignment
Assignment: Assessing and Treating Pediatric Clients With Mood Disorders
When pediatric clients present with mood disorders, the process of assessing, diagnosing, and treating them can be quite complex. Children not only present with different signs and symptoms than adult clients with the same disorders, but they also metabolize medications much differently. As a result, psychiatric mental health nurse practitioners must exercise caution when prescribing psychotropic medications to these clients. For this Assignment, as you examine the client case study in this weeks Learning Resources, consider how you might assess and treat pediatric clients presenting with mood disorders.
Note: This Assignment is the first of 10 assignments that are based on interactive client case studies. For these assignments, you will be required to make decisions about how to assess and treat clients. Each of your decisions will have a consequence. Some consequences will be insignificant, and others may be life altering. You are not expected to make the right decision every time; in fact, some scenarios may not have a right decision. You are, however, expected to learn from each decision you make and demonstrate the ability to weigh risks versus benefits to prescribe appropriate treatments for clients.

Due to her limited English speaking abilities, she is unable to provide specific details as to her complaints but the shelter employee states that she has recently stopped eating and has not looked well for the past couple of days.
Based on the limited information provided, please answer the following questions.
How will you prioritize your care of Amira, what assessments will you complete, and in what order? Please provide rationale for choosing this order.
Are there any cultural beliefs/practices that must be taken into consideration when planning her care?
Considering her symptoms of abdominal pain and bleeding, is it possible that her status as a homeless refugee is a causative or contributing factor to her illness? Please provide rationale for your response.
Due to her limited English speaking abilities, she is unable to provide specific details as to her complaints but the shelter employee states that she has recently stopped eating and has not looked well for the past couple of days.

Assignment:Gathering Data in Health Care
You only need to post an initial response to one of the following. Your participation posts can be to anyone and either option. The only rule regarding your selection is what others have posted. Half of your group will respond to option #1 and half of your group will respond to option #2. Therefore if you are in a group of 6 and when you log on 3 people already made a post to option #1 than you will make your initial post to option #2. Your posts to others can be in either option #1 or #2.
Option 1: In the following scenario, discuss the developmental, age, socioeconomic and cross-cultural considerations that should be considered during the gathering of the subjective and objective and the provision of health care. Discuss the findings presented. Is there additional information / assessments you would like to know prior to making a clinical judgment?
It is December in a northern state. S.A. has brought her two children, ages 3 and 5 years, to the health clinic for the first time. She states that she recently emigrated from India. She is wearing a sari with a light coat, and each child is wearing a light jacket. Interaction between parent and children is appropriate. The children both fall at the 4th percentile for both height and weight. Overall, nutrition status appears to be satisfactory.
Option 2: S.P. is a 75-year-old female who presents to the providers office with fatigue. Subjective data includes: PMH hypertension, hyperlipidemia, MI 3 years ago; fatigue started about a month ago, getting worse; relieved with rest, exacerbated with activity; denies chest pain, ankles swollen. The Objective data includes: Vital signs T 98.3, P 112, R 18, BP 110/54; Lungs bilateral lower lobe crackles anterior and posterior; O2 sat = 94% on RA; Skin is cool to touch; CV- heart rate regular, positive peripheral pulses, ECG=no changes, +2 edema bilateral ankles; Medications include Metoprolol 20mg/day and Aspirin 325mg per day.
1. What other questions should the nurse ask about the fatigue?
2. What other assessments are necessary for this patient?
3. Based on the readings, what is the most likely cause of fatigue for this patient?

Week 2 Project
Assessing and Planning Care for an Elderly Person
In order to gain an insight into the world of elder adults, it is important to understand how they view themselves and the values they hold. Additionally it is important to assess and determine his/her needs and establish appropriate interventions for this individual.
By the due date assigned, choose an older adult to interview. This cannot be a patient in your clinical setting. You can use a friend, family member, or co-worker. The older adult should be 65 years or older. Use the format provided to record the responses. A list of questions is available for you to start with. Include 23 questions of your own to get a complete picture of the older adult. Summarize your findings and also contrast the responses with findings in your readings and other current literature.
Download a patient questionnaire. This form should be used as an example.
After gaining permission, conduct a physical and mental functional assessment of the older adult you have chosen. Review your readings for the process of functional assessment.
Make use of the tools discussed this week to complete a comprehensive assessment of your patient. Search the Internet for resources on these tools.
Tinetti Balance and Gait Evaluation
Katz Index of Activities of Daily Living
Assessment of Home Safety
The Barthel Index
Make sure the older adult is clearly identified on the tools. Do not include their name, but do include professional or other designation, and age. Your name should also be identified on the tool. (This should be a part of your Appendix.)
Compare and contrast the age-related changes of the older person you interviewed and assessed with those identified in this weeks reading assignment. Identify a minimum of three comprehensive interventions for each problem.
Make sure that you integrate personal cultural awareness and cultural competency.
Paper should be 56 pages, not including the questionnaire or tools used. These should be attached as an Appendix. Remember to use headings to identify the different sections in your paper.
