The patients are from Villa Siena, which is a senior living community at Mountain View. They are sponsored by the Daughters of Charity of St. Vincent de Paul and are open to seniors aged sixty years and older. Most patients population is suffering from Dementia. Most patients are wheelchair bonded. For the interventions, using some stretching exercises for wheelchair bond patients during their activities.1. This week you will consider nursing interventions for key health concerns in your population. Develop evidence-based, culturally appropriate, measurable (SMART) interventions to help your population members maintain an optimal state of health?2. Include the 3 levels of prevention (primary, secondary, and tertiary) in your goals. Is there a possibility of changing the behavior or social determinants of health to improve conditions?
