Failure Mode and Effects Analysis

1.     Create the FMEA template in Excel. 2.     Create a process map for this project and use it for this assignment. 3.     Using the at least 3 of the individual process steps, identify potential failure mode and associated causes for the failure. 4.      Rate each cause for severity, likeliness of occurrence, and ease of detection. 5.     Determine the Risk Priority Number (RPN). 6.     Identify recommended actions for the top three RPN along with person responsible and target date of completion.
