Family Centered Care
Consider the importance of patient and family centered care as it relates to care coordination (think of engaging the patient and family in developing and understanding the plan of care [i.e who will deliver care? When? What is their role?]. Choose one patient from this week’s scenario to discuss various evidence-based patient engagement strategies and select a specific strategy you can utilize to assess and activate patient and family involvement in care.
