[ORDER SOLUTION] Health Care Disparities

1.  Discuss how care coordination, case management and population health management can be integrated by the Nurse Practitioner to optimized patient and community health. Discuss the differences between the functions of these concepts, and what barriers exists at the team and system levels that continue to challenge these processes from being successful.or2.  Discuss the effect of Health Care coverage on health and health care disparities in the US. How has the Affordable Health Care Act (ACA) affected the practice patterns of providers in primary care in the United States in the past five years? Is there any evidence that the ACA has benefited any specific population? What are the most significant changes being proposed by the current government to the ACA as it is currently written?
