Middle Range Nursing Theories
Middle-range theories have justifiably come out as the focal point of knowledge development in nursing. They have influenced a wide acceptance of the need to establish middle-range theories to support nursing practice. Middle-range theories are increasingly being developed and examined through practice-oriented nursing research. They have a narrow scope and concentrate on a specific phenomenon that reflects on nursing practice. Middle-range theories that support my philosophy of nursing practice include Penders Health Promotion Model and Kolcabas Comfort Theory.
The health promotion model was developed to guide nurses in assisting clients in attaining better health, enhanced functional ability, and improved quality of life. The model identifies behavior-specific cognitions and affects as the primary motivational mechanisms for health promotion behavior (Aqtam & Darawwad, 2018). They include

perceived benefits of action, self-efficacy, barriers to action, activity-related affect, and interpersonal and situational influences. Besides, the model is justified by the need for behavioral and environmental changes to improve a society where lifestyle factors influence numerous health problems. It is rooted in established theories of human behavior, such as the expectancy-value theory and social-cognitive theory (Aqtam & Darawwad, 2018). The model assumes that various patient characteristics and cognitive-affective factors combine with competing needs and preferences and a commitment to a plan of action to elucidate the possibility of health-promoting behavior.
The Comfort theory explains a philosophy of care through which the holistic comfort needs of patients, families, and nurses are acknowledged and addressed. The theory justifies that the desired outcome of this type of care is improved comfort, which is a compassionate and patient-centered goal (Peterson & Bredow, 2019). Enhanced comfort is connected to successive desirable outcomes such as increased patient or nurse function, reduced hospital stays, reduced readmissions, increased patient satisfaction with care, the longevity of employment, and improved cost-benefit ratios for the organization (Peterson & Bredow, 2019). The patient needs are modified by dominant variables, which are factors that the nurse cannot alter. The nurse creates a comfort care plan based on the patients needs, which seeks to improve comfort. When the patients comfort is enhanced, they can participate more in health-seeking behaviors.
Week 1: Philosophy of Nursing Practice

Through the Discussion this week, and the Assignment you will begin, you will articulate your philosophy of nursing practice as you look for insight into what shapes your philosophy and contrasting philosophies of others. That understanding can be invaluable in your future work as a DNP leader advocating for changeoften difficult for individuals and organizationsthat promotes positive outcomes for patients and the larger society.

Discussion: Philosophy of Nursing Practice
For this first course Discussion, you will present a philosophy of nursing practice that draws on your experience, area(s) of expertise, and beliefs. Looking ahead to your role as a DNP, also consider how to expand your awareness of issues beyond your own current practicesuch as regarding diversity, global health, collaborationand how to fulfill the Walden requirement to advocate for social change.

In short, begin now to commit to a higher level of contribution and practice as a DNP and to reflect that commitment in your philosophy of nursing practice for today and the future. As you continue through the course, note this image will appear with each Discussion and indicates an opportunity to connect your thinking with that of colleagues, for learning with and from one another.
To prepare:
Review the Week 1 Learning Resources. Pay particular attention to the Chapter 1 reading from McEwen and Wills, Philosophy, Science, and Nursing.
Reflect on your nursing experience, expertise you have developed, and beliefs about nursing and nursing practice you have formed as a result.
Consider how your philosophy of nursing practice is shaped by these elements.
Consider goals for expanding your professional awareness through the DNP program and the requirement as a Walden student to be an advocate for social change. How can your philosophy of nursing practice support you in these areas? How can these goals and Walden social change requirement enrich your philosophy of nursing practice?
With these thoughts in mind
By Day 3 of Week 1
Post an explanation of your philosophy of nursing practice that briefly describes your nursing experience and area(s) of expertise. Be sure to explain your beliefs that inform your philosophy of nursing practice and your goals for expanding your experience and practice as a DNP (e.g., embrace of diversity, global health issues, collaboration). Then, explain the DNP role as a social change agent and recommend at least one way you will advocate for positive social change as a Walden DNP. Be specific and provide examples, with all citations in APA 7 style.
Read a selection of your colleagues posts.
By Day 6 of Week 1
Respond to at least two colleagues on 2 different days. Choose colleagues with different life experiences and beliefs from your own, and contrast how these factors have shaped their philosophies compared with yours. Also comment on their goals for expanding their experience, practice, and social change advocacy with suggestions or guidance based on your experience. Be specific and provide examples.

Week 1 Discussion
An event in my career as a nurse that I will never forget was my experience as a hospice nurse, which was a very challenging job at times. One patient I will never forget was a young woman with terminal pancreatic cancer. I went to work and got my report from the other nurse that we had a new admission. When she stated the name of the patient and gave the report on her, I could not believe what I was hearing. Soon after the report I went to visit the patient in her room to see for myself if it was the person, I thought it was. As I walked in and recognized the patient, my heart sank seeing my sons fourth grade teacher in bed. I froze for a second, we hugged and cried together. She looked at me and said, I am thankful I am in good hands but please promise me not to tell your son that I am dying, I nodded my head.
Nursing is a Profession concerned with the provision of services essential to the maintenance and restoration of health by attending to the needs of sick persons (Patterson et al., 2021). Nursing is a science that requires in-depth knowledge, skills and understanding. However, for me nursing goes beyond that; it is caring for an individual, irrespective of age, whole heartedly, both physically and mentally. Nursing is also about helping others in a compassionate, safe and accountable way while being non-judgmental to the patients needs regardless of race, spiritual beliefs, lifestyle choices, financial status, or disability (Patterson et al., 2021). Furthermore, nursing is about empowering patients, guiding them toward recovery and supporting them in times of need. Finally, nursing is educating people about prevention of illness and the importance of staying healthy through making smart choices.
Dr. Jean Watson is a nursing theorist who influences my practice. Dr. Watson developed the theory of human caring between 1975 and 1979 (Wei et al., 2020). Her theory looks deeper into the meaning of nursing and goes beyond the physical worldview of nursing. Her theory defines caring as a science and seeks to understand the connection between art, humanities, philosophy, spirituality, science and technology (Wei et al., 2020). As a nurse, I use the above assumptions every day and they have molded me into a more accepting person and to see the best in a person. I have a strong belief that it is a must for a nurse to be caring, and that caring is key to improving effective and positive nursing outcomes. Finally, having traits such as caring, loving, forgiving and compassion for patients ultimately leads to a better care experience for patients.
DNPs translate research evidence into practice, whereas the PhD discovers the evidence (Ahmed, 2018). We need both types of doctorates. I view what a DNP does as implementation science. According to the American Association of Critical-Care Nurses (AACN), APRNs prepared at the doctoral level possess a blend of clinical, organizational, economic, and leadership skills that allow them to critique nursing practice and design programs of care delivery that can have a significant positive impact on healthcare outcomes (Ahmed, 2018). The DNP does not alter the actual scope of practice for nurse practitioners and other advanced practice registered nurses. Instead, DNPs work alongside their masters-prepared colleagues in an expanded role in which they serve as leaders, change agents, and evaluators of care. This is especially true in the current environment we live in where diversity and inclusion are key factors when evaluating quality and outcomes of care.
I have accepted a job offer at a position where I will work with individuals from an underserved community. I consider this to be a huge privilege and opportunity to leave a positive mark on the community in which I will serve. I truly want to be an agent of change as well as a beacon of light for those who dare to dream. Given how unique my own story is, sharing it with this community will help improve the quality of care and outcomes of the individuals I look forward to working with.

Week 2 assignment
One of the things that I like to focus on when I am treating my patients in my clinic is treating the whole individual: I dont just give medications and scripts, but I also try to incorporate a lot of teaching into my practice. I try to give enough time to my patients in the interest of addressing issues that will affect the outcome of their health. Sometimes things that will also affect their diabetes status may not just be medication compliance but also counseling on proper nutrition and how to avoid bouts of hunger that will contribute to them deviating from their diet. That may also involve their support system from their loved ones or lack thereof. I try to incorporate a more holistic approach to their care, and that encompasses biological, psychological, sociocultural, and spiritual dimensions (Engebretson and Littleton 2001). I feel it is important to not only use polypharmacy but other aspects to improve health as well.
It is important to be empathetic with our patients as we treat them. We as providers do have a reasonability to them (Andershed and Olsson 2009) We need to value them as people and incorporate caring into our plans. I feel if we treat the person as a whole and live a loved one we can greatly improve the health as a result.
