Discuss what additional aspects mangers and leaders would need to initiate in order to ensure professionalism throughout diverse health care settings while addressing the selected issue.
Describe a that would best address the chosen issue. Explain why this style could be successful in this setting.
Re: Topic 2 DQ 3
A medication error occurs when a healthcare provider, patient, or consumer makes a mistake with a medicine and, as a result, the medication is misused or the patient is injured (Hughes & Blegen, 2008). Nurses are constantly challenged to ensuring that patients receive the proper drug at the right time due to poor nursing education, heavy workloads and staffing deficiencies, defective dispensing methods, and difficulties with labeling of drugs (Hughes & Blegen, 2008). Improved communication in the medicine delivery process will be the focus of the planned quality improvement program. Medication errors will be significantly reduced, patient mortality and morbidity will be reduced, communication between health care providers and their patients will be improved, continuous error monitoring will be ensured, clinicians will have information tools and medication labeling will be standardised, and these are all goals of the initiative. Medication mistakes can happen in any context, but the risks of adverse drug events are higher when medications are delivered in specialty settings or with complicated dose regimens. Those who work in the delivery unit at Southern New Hampshire Medical Center, including pharmacy and administrative employees, will be the primary audience. Patients are at risk of injury if drug therapy is used incorrectly, which is increasingly common as the primary treatment for most disorders. It is only if this project is a success that patients will reap the benefits of mediation in the form of better health care and fewer mistakes (Hughes & Blegen, 2008). Patients safety will be protected by only using proven methods, thanks to the program.https://nursingpaperslayers.com/nrs-451-topic-2-roles-and-responsibilities-in-leadership-and-management-tasks/
This project will necessitate interprofessional cooperation. Registered nurses on the floor, clinical supervisors, unit managers, physicians, pharmacy staff and pharmacist, director of education, director of research, and head of nursing services will be part of the

high-performance and successful team. The members of this group will be participating at every stage of the project. Patients will be a part of the implementation and evaluation processes because their input is critical. This projects budget will be justified by the reality that todays healthcare is both value- and cost-driven. Healthcare providers are constantly working to deliver the best possible care at the lowest possible cost. Hospitals are willing to pay the price to undertake quality improvement programs in order to maximize payment for the quality of services they give. There is a need for this endeavor because of an increase in drug mistakes.
the total number of medication administration errors made by nurses; the rate over the study; the number of errors related to high alert medications like Pitocin, potassium chloride, and magnesium sulfate; the number and rate of errors related to the administration route, the wrong drug, wrong patient, wrong time, and time of the occurrence; and the time of the occurrence of the errors. The degree to which patients and healthcare professionals (such as nurses, doctors, and pharmacists) are pleased with the initiative that was put in place. When it comes to patient and nurse satisfaction, it will be based on the ease with which the process is implemented, as well as how much time is saved. Re: Topic 2 DQ 3
A medication error is any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is under the healthcare professionals, patients, or consumers control (Hughes & Blegen, 2008). With insufficient nursing education about patient safety and quality, excessive workloads and staffing shortages, flawed dispensing systems, and drug labeling issues, nurses are constantly challenged to ensure that patients receive the right drug at the right time (Hughes & Blegen, 2008). The proposed quality improvement initiative will focus on reducing medication errors through improved communication during the medication administration process. The initiatives goals will be to significantly reduce medication errors, lower patient mortality and morbidity, improve communication between providers and patients, ensure continuous error monitoring, provide clinicians with information tools, and standardize medication labeling and drug-related information. Medication errors occur in all settings, but those with complex dosing regimens and administered in specialty areas have an increased risk of adverse drug events. The target population will be healthcare professionals, including pharmacy staff and administrative staff at Southern New Hampshire Medical Centers delivery unit. Medication therapy is increasingly being used as the primary intervention for most illnesses, exposing patients to potential harm when errors occur. The success of this initiative will ensure that patients only benefit from mediation through effective management of their illnesses and better outcomes, with few, if any, errors (Hughes & Blegen, 2008). In addition, the initiative will ensure that only evidence-based practices are used to ensure patient safety. This project will necessitate interprofessional collaboration. Registered nurses on the floor, clinical supervisors, unit managers, physicians, pharmacy staff and pharmacist, director of education, director of research, and head of nursing services will be part of the high-performance and effective team. This group will be present throughout the process. Patients will be involved in the implementation and evaluation phases because their feedback is important. This projects budget will be justified by the fact that healthcare today is value-based and cost-driven. Healthcare organizations strive to provide the best care at the lowest possible cost. With reimbursement for service quality, hospitals are willing to pay the price to implement quality improvement projects that will ensure maximum reimbursement. Increased drug errors indicate poor quality care, necessitating this initiative. NRS 451 Topic 2: Leadership and Management Roles and Responsibilities
The total number of medication administration errors made by nurses; the rate of the errors over the study period; the number and rate of errors related to high alert medications such as Pitocin, potassium chloride, and magnesium sulfate; the number and rate of errors related to omission, administration route, wrong drug, wrong patient, and wrong time; the time of the occurrence and identification of the errors; the time of the occurrence and identification of the errors Patient and clinical staff (nurses, physicians, and pharmacy staff) satisfaction with the implemented initiative. The level of satisfaction will be determined by how user-friendly the process is, the amount of time saved, and how safe both nurses and patients feel during the medication administration process.
