[SOLVED] A Formal Letter of Appeal

I’m trying to study for my English course and I need some help to understand this question.

i was very sick the last couple of weeks i didn’t leave the apartment. I was vomiting everything i ate, with extreme pain in my upper right abdomen. i was diagnosed with a severe gastrointestinal reflex disease and an peptic ulcer, it was the worst month of my life., i went back home since i was living alone in my apartment,because my family was really worried. I am really depressed that i missed a lot of classes. I would put all my effort to replace all the work i missed in my classes. This letter is for approval for a medical withdrawal,i attended only the first week.
